The Bad Impact of Proteinuria on Kidneys
Proteinuria is not only a sign of glomerular damage but also a factor which leads to the fibrosis of nephridial tubules and mesenchyma. but how does the damage happen when patients have proteinuria? I'd like to tell you something about it.

As moderate-molecular protein dominates in proteinuria, I’d like to introduce something about how moderate-molecular protein leads to kidney damage.
Moderate-molecular protein include albumin and transferrin. Albumin which is the main cause of tubulointerstitial fibrosis accounts for 50%-60% in glomerular proteinuria.
Albumin can stimulate the functional renal cells to produce chemotactic factor, which lead to the inflammatory reaction. Besides, albumin can combine with many substances in humane body like fatty acid to form toxic metabolin in kidney, which can lead to damage to renal tissues and renal cells. All these conditions can cause kidney disease.
Transferrin can combine with iron taken in by human body and when the combination goes through kidneys, it releases the iron, which can cause damage to renal mesenchyma. What’s more, transferrin can cause the hyperplasia of mesangial cells and then produce more extracellular matrixes. Therefore, as the transferrin in urine increases, it may lead to renal fibrosis and then decline in renal function.
